A drug-free way to treat depression and OCD using targeted magnetic pulses. No sedation, no systemic side effects, and you can drive home immediately after treatment. Board-certified psychiatrists at two nearby Nassau County locations.

Some patients arrive here because the medications simply stopped working. Others tried three or four options and couldn’t tolerate the side effects. And a growing group of patients want relief from depression without ever taking a psychiatric medication in the first place. For all three groups, TMS therapy in Garden City opens a different door.
Transcranial Magnetic Stimulation uses focused magnetic pulses to activate brain regions responsible for regulating mood. Where oral antidepressants act on your entire body chemistry, TMS works directly on the neural circuits involved in depression itself. The treatment is FDA-approved, insurance-covered for standard protocols, and supported by decades of evidence showing 70-80 percent of patients respond. At Anxiety & Depression Psychiatry, our board-certified team provides TMS from both Nassau County offices, often coordinated with our ketamine therapy service in Garden City, with additional resources through the National Institute of Mental Health.
We offer three protocol formats: standard TMS, accelerated TMS, and one-day TMS.
Depression is linked to reduced activity in the left dorsolateral prefrontal cortex (DLPFC), a brain region central to mood regulation, motivation, and emotional processing. TMS delivers precisely targeted magnetic pulses to this underactive region, stimulating neurons and promoting neuroplasticity — your brain’s ability to form new connections and shift out of stuck patterns of activity.
A helpful way to picture it: depression carves narrow grooves in your brain’s circuitry, pathways that keep firing along the same unhelpful lines. TMS helps the brain reroute around those grooves by activating new neural pathways. Over the course of treatment, those new patterns stabilize and produce lasting change.
The mechanism is fundamentally different from what oral antidepressants do. Medications shift neurotransmitter levels across your entire nervous system, which is why they produce systemic side effects. TMS acts locally on the specific brain region involved in depression, without touching the rest of your physiology.
Research published in General Psychiatry documents response rates of 40 to 60 percent in patients with treatment-resistant depression, with remission rates of 30 to 50 percent. Naturalistic follow-up data shows benefits persist for most responders well past the treatment course, with many patients maintaining improvement without additional intervention.
TMS received FDA approval for treatment-resistant depression in 2008 and has since become a mainstream treatment option. It’s particularly valuable for patients who haven’t responded to antidepressants or can’t tolerate medication side effects. TMS also works for patients experiencing their first depressive episode who prefer a non-medication approach.
The FDA approved TMS for obsessive-compulsive disorder in 2018, making it one of the few non-medication treatments available for this challenging condition. TMS for OCD typically targets different brain regions than depression treatment, focusing on areas involved in the rigid, repetitive thought patterns that characterize OCD.
In 2021, the FDA cleared TMS specifically for major depressive disorder with anxiety symptoms. This is significant because anxious depression often responds differently to treatment than depression alone, and many patients struggle to find effective relief.
While FDA approval currently covers depression and OCD, research supports TMS effectiveness for PTSD, bipolar depression (for the depressive phase), and other conditions. We evaluate patients individually to determine whether TMS might help their specific situation.
Three protocol formats accommodate different clinical situations, schedules, and treatment goals. Each has distinct advantages depending on your case.
Standard TMS (repetitive transcranial magnetic stimulation, or rTMS) is the original FDA-approved protocol with the longest track record and the strongest research base. It runs as one daily session, five days a week, across roughly six weeks.
Each session delivers high-frequency magnetic pulses (typically 10 Hz) to your left DLPFC. The stimulation activates neurons in the underactive region and progressively restores normal activity patterns. Session length ranges from 19 to 37 minutes depending on protocol parameters.
Standard sessions deliver 3,000 pulses each. Across a full 36-session course, patients receive over 100,000 total pulses, which is the cumulative stimulation that produces durable changes in brain function.
Treatment Schedule:
Session Experience: You’ll sit in a comfortable chair while we position the TMS coil against your scalp. The device delivers magnetic pulses that feel like tapping on your head. Most patients read, watch videos on their phone, or simply relax during sessions. You’re awake and alert throughout.
Timeline for Results: Most patients begin noticing improvement within two to four weeks. Benefits typically continue building throughout the treatment course and often persist for months to years after completion.
Standard TMS is ideal if you can commit to regular brief appointments over several weeks, want the most established and researched protocol, need insurance coverage (standard TMS is covered by most major insurers), prefer a gradual treatment approach that fits around work, or have moderate rather than severe depression.
Many patients schedule sessions before work, during lunch, or at the end of the day without significant disruption to their routine. There’s no sedation or recovery period, so you can drive yourself and return to normal activities immediately.
Standard TMS is FDA-approved and covered by most major insurance plans for treatment-resistant depression. Coverage typically requires documentation that you’ve tried other treatments (usually at least one antidepressant) without adequate success. Our team handles benefits verification and prior authorization.
Accelerated TMS compresses weeks of treatment into a single work week by delivering multiple sessions per day. Our protocol delivers 50 total sessions across five consecutive days, adapted from the Stanford Accelerated Intelligent Neuromodulation Therapy (SAINT) protocol, which produced landmark results in clinical trials.
The format works best for patients who need faster results, can take a full week away from work, or haven’t responded adequately to standard TMS.
Stanford researchers developed SAINT after recognizing that six weeks is often too long to wait for severely depressed or actively suicidal patients. Their approach uses intermittent Theta Burst Stimulation (iTBS), a newer form of TMS that delivers therapeutic pulses in concentrated bursts. Each iTBS session takes about three minutes rather than 19 to 37, but research shows equivalent clinical effectiveness.
A 2024 meta-analysis published in BMC Psychiatry examined accelerated TMS studies and found significant depression score reduction post-treatment, with no meaningful difference between accelerated and standard protocols. The analysis also documented sustained benefit at follow-up, with depression scores remaining lower months after treatment concluded.
Stanford’s original SAINT research reported response rates approaching 90 percent with remission rates near 80 percent in treatment-resistant patients. A 2024 pilot study published in Brain Stimulation using a pragmatic accelerated protocol reported 70 percent response and 55 percent remission at four-week follow-up, with effect sizes almost three times what’s typically classified as “large.”
Treatment Schedule:
A Typical Treatment Day: You’ll arrive in the morning and receive your first session. After each treatment, you’ll have about 50 minutes before your next session. Many patients work on laptops, make phone calls, read, or relax between sessions. Lunch is taken between sessions. You’ll finish in the late afternoon.
The five-day schedule typically runs Monday through Friday. You’ll need to arrange transportation for the treatment week since we recommend not driving during the intensive treatment period.
Timeline for Results: Many patients notice improvement within the first few days of treatment. Research shows depression scores typically decrease progressively each treatment day. Benefits are often well-established by the end of the five-day protocol and maintained at follow-up.
Accelerated TMS Long Island treatment is ideal if you need faster results than standard TMS provides, can dedicate one full week to treatment, have severe depression requiring rapid intervention, are traveling from out of state or internationally and can’t commit to six weeks of daily treatment, have tried standard TMS without adequate response, or work in a demanding profession where six weeks of daily appointments isn’t feasible.
The accelerated protocol requires more intensive time commitment during the treatment week but dramatically reduces total treatment duration. Instead of 36 visits over nine weeks, you complete treatment in five days.
Time Off Work: You’ll need to take the treatment week off from work or other major responsibilities. Many patients find this easier to arrange than six weeks of daily appointments that interrupt their schedule.
Accommodations: For patients traveling from outside Long Island, comfortable hotels are available near both our Great Neck and Mineola offices. We’re close to JFK and LaGuardia airports and easily accessible via Long Island Railroad.
Post-Treatment: After completing the five-day protocol, you can typically return to normal activities. We schedule follow-up appointments to monitor your response and determine if any maintenance treatment would be beneficial.
Accelerated TMS costs $12,500 for the complete five-day protocol. Most insurance plans don’t cover accelerated protocols yet, though this is changing as research continues to demonstrate effectiveness. We can provide documentation if you wish to submit for out-of-network reimbursement.
One-Day TMS (ONE-D) is the most condensed TMS format offered, delivering 20 iTBS sessions in a single day alongside medications designed to enhance neuroplasticity. It’s built for patients who need the absolute fastest treatment timeline available.
The ONE-D protocol builds on the same iTBS foundation as accelerated TMS but compresses the treatment window further. You receive 20 iTBS sessions spaced across a single day, timed to allow the brain to respond between each stimulation.
What makes ONE-D distinct is the addition of two medications that temporarily heighten neuroplasticity — your brain’s ability to form new connections. The goal is to maximize the plastic changes TMS produces during a brief, intensive treatment window.
One-day protocols represent the current frontier of TMS research. The ONE-D combination isn’t yet FDA-approved as a specific protocol, though its underlying components are established. Research has shown that multiple daily sessions are safe and effective, neuroplasticity-enhancing medications can meaningfully augment TMS effects, and compressed protocols can produce rapid improvement.
Early data from single-day intensive protocols is promising, though this approach has less long-term follow-up data than standard or five-day accelerated formats.
Treatment Schedule:
The Treatment Day: You’ll arrive in the morning after following preparation instructions regarding the plasticity-enhancing medications. Throughout the day, you’ll receive iTBS sessions with breaks between treatments. Our team monitors you throughout the day and ensures your comfort. You’ll need someone to drive you home after treatment.
Timeline for Results: Some patients notice improvement within days of ONE-D treatment. As with all TMS protocols, full benefits may continue developing over the following weeks as brain changes stabilize.
One-Day TMS Long Island treatment may be right if you need the absolute fastest treatment timeline, are a busy professional who can’t take extended time away from work, are traveling from far away and have very limited time available, are comfortable with a newer protocol that has less long-term data, or want to combine TMS with plasticity-enhancing approaches.
Newer Protocol: ONE-D is based on sound scientific principles and emerging research, but it has less extensive data than standard or five-day accelerated TMS. We discuss this openly during consultation so you can make an informed decision.
Not FDA-Approved: While the individual components (iTBS and the medications used) are established, the specific ONE-D combination protocol hasn’t received separate FDA approval. This is common for innovative treatment approaches that combine existing approved elements in new ways.
Follow-Up: After ONE-D treatment, we monitor your response closely. Some patients benefit from additional sessions or maintenance treatment.
One-Day TMS costs $6,000 for the complete protocol. Insurance typically doesn’t cover this approach. We can provide documentation for out-of-network reimbursement submission if desired.

You’ll meet with one of our psychiatrists for a comprehensive evaluation. We review your treatment history, current symptoms, and treatment goals to determine whether TMS is appropriate and, if so, which protocol format fits best. We also verify insurance coverage for standard TMS, including prior authorization, which our team manages entirely.
The first session includes motor threshold determination, the calibration step that identifies the minimum magnetic intensity needed to activate your motor cortex. This individualized calibration is what allows treatment to match your specific neural responsiveness.
You’ll sit in a comfortable treatment chair while the coil is positioned against your scalp. The magnetic pulses feel like tapping. The sensation is unfamiliar at first but most patients adjust within the first session or two. You stay awake and alert throughout.
TMS has an excellent safety profile compared to psychiatric medications. The most common side effect is mild scalp discomfort or headache during or shortly after treatment, which typically responds to over-the-counter pain relievers and often diminishes as the treatment course progresses.
Unlike antidepressants, TMS doesn’t cause weight gain, sexual dysfunction, emotional blunting, or withdrawal. It’s safe for elderly patients and doesn’t create drug interactions.
Benefits typically persist for months to years after the treatment course concludes. If symptoms return, treatment can be repeated. Most insurance plans cover additional courses after a period of sustained benefit. Some patients maintain results with occasional maintenance sessions.

Results vary by protocol. Standard TMS typically shows improvement within two to four weeks. Accelerated TMS often produces noticeable improvement within the five-day treatment period. One-day TMS may show effects within days. All protocols continue building benefits after treatment completion.
TMS isn’t painful, though the magnetic pulses create a tapping sensation on the scalp that some patients find unusual at first. Mild scalp discomfort or headache can occur but typically diminishes as treatment progresses.
Yes, TMS can be safely combined with most psychiatric medications, and some antidepressants may actually enhance TMS effectiveness. We’ll review your complete medication list during your evaluation.
TMS benefits can last months to years after treatment completion. If symptoms return, TMS can typically be repeated. Some patients benefit from periodic maintenance sessions.
Standard TMS is covered by most major insurers for treatment-resistant depression. Accelerated and one-day protocols are typically self-pay.
Standard TMS delivers one session daily over six weeks (36 total sessions). Accelerated TMS delivers 10 sessions daily over five days (50 total sessions). Research shows similar effectiveness, but accelerated TMS produces faster results. Standard TMS is usually covered by insurance; accelerated TMS typically isn’t.
TMS isn’t appropriate for people with metal implants in or near the head (except dental fillings), cochlear implants, or implanted stimulators. People with a history of seizures require careful evaluation. We screen for contraindications during your initial consultation.
For standard TMS, yes. You can drive immediately after sessions. For accelerated and one-day protocols, we recommend arranging transportation during the treatment period due to the intensive schedule.
