A medication-free approach to treating depression and OCD. No sedation, no systemic side effects, and you can drive home right after treatment. Board-certified psychiatrists at two convenient Nassau County locations.

Maybe antidepressants helped for a while but stopped working. Maybe the side effects were intolerable. Or maybe you’d simply prefer not to take psychiatric medication at all. Whatever brought you here, TMS therapy on Long Island offers a genuinely different approach to treating depression.
Transcranial Magnetic Stimulation uses focused magnetic pulses to stimulate specific brain regions involved in mood regulation. Unlike medications that affect your entire body and come with side effects like weight gain, sexual dysfunction, or emotional blunting, TMS targets the problem at its source. The treatment is FDA-approved, covered by most insurance plans, and backed by decades of research showing 70-80% of patients respond to treatment.
At Anxiety & Depression Psychiatry, our board-certified psychiatrists provide TMS therapy Long Island patients can access at our Great Neck and Mineola offices. We offer three protocol options to fit different needs and schedules: standard TMS, accelerated TMS, and one-day TMS.
Depression involves reduced activity in the left dorsolateral prefrontal cortex (DLPFC), a brain region crucial for mood regulation, motivation, and emotional processing. TMS delivers magnetic pulses to this underactive area, stimulating neurons and promoting neuroplasticity, your brain’s ability to form new connections and healthier patterns of activity.
Think of it this way: depression creates ruts in your brain’s circuitry, pathways that keep firing in unhelpful patterns. TMS helps lift your brain out of those ruts by stimulating new neural activity. Over the course of treatment, these changes become more stable, leading to lasting improvement in mood and function.
The mechanism differs fundamentally from antidepressants. Medications work by altering neurotransmitter levels throughout your entire body, which explains why they cause systemic side effects. TMS works locally, directly on the brain region involved in depression, without affecting the rest of your system.
Research published in General Psychiatry found that 40-60% of patients with treatment-resistant depression respond to TMS, with remission rates of 30-50%. A large naturalistic study following patients for a year after treatment found that benefits persisted for most responders, with many maintaining improvement without additional treatment.
TMS received FDA approval for treatment-resistant depression in 2008 and has since become a mainstream treatment option. It’s particularly valuable for patients who haven’t responded to antidepressants or can’t tolerate medication side effects. TMS also works for patients experiencing their first depressive episode who prefer a non-medication approach.
The FDA approved TMS for obsessive-compulsive disorder in 2018, making it one of the few non-medication treatments available for this challenging condition. TMS for OCD typically targets different brain regions than depression treatment, focusing on areas involved in the rigid, repetitive thought patterns that characterize OCD.
In 2021, the FDA cleared TMS specifically for major depressive disorder with anxiety symptoms. This is significant because anxious depression often responds differently to treatment than depression alone, and many patients struggle to find effective relief.
While FDA approval currently covers depression and OCD, research supports TMS effectiveness for PTSD, bipolar depression (for the depressive phase), and other conditions. We evaluate patients individually to determine whether TMS might help their specific situation.
We offer three TMS protocols to match different needs, schedules, and treatment goals. Each approach has distinct advantages depending on your situation.
Standard TMS (also called rTMS or repetitive transcranial magnetic stimulation) is the original FDA-approved protocol with the longest track record and most extensive research support. This approach involves one daily treatment session, five days per week, for approximately six weeks.
Each session delivers high-frequency magnetic pulses (typically 10 Hz) to your left dorsolateral prefrontal cortex. The stimulation activates neurons in this underactive brain region, gradually restoring normal activity patterns over the course of treatment. Sessions last 19-37 minutes depending on the specific protocol parameters.
The standard protocol delivers 3,000 pulses per session. Over a typical course of 36 sessions, you receive over 100,000 total pulses, enough cumulative stimulation to produce lasting changes in brain function.
Treatment Schedule:
Session Experience: You’ll sit in a comfortable chair while we position the TMS coil against your scalp. The device delivers magnetic pulses that feel like tapping on your head. Most patients read, watch videos on their phone, or simply relax during sessions. You’re awake and alert throughout.
Timeline for Results: Most patients begin noticing improvement within two to four weeks. Benefits typically continue building throughout the treatment course and often persist for months to years after completion.
Standard TMS is ideal if you can commit to regular brief appointments over several weeks, want the most established and researched protocol, need insurance coverage (standard TMS is covered by most major insurers), prefer a gradual treatment approach that fits around work, or have moderate rather than severe depression.
Many patients schedule sessions before work, during lunch, or at the end of the day without significant disruption to their routine. There’s no sedation or recovery period, so you can drive yourself and return to normal activities immediately.
Standard TMS is FDA-approved and covered by most major insurance plans for treatment-resistant depression. Coverage typically requires documentation that you’ve tried other treatments (usually at least one antidepressant) without adequate success. Our team handles benefits verification and prior authorization.
Accelerated TMS compresses weeks of treatment into days by delivering multiple sessions per day. Our protocol delivers 50 treatments over five consecutive days, based on the groundbreaking Stanford Accelerated Intelligent Neuromodulation Therapy (SAINT) protocol that achieved remarkable results in clinical trials.
This approach is ideal for patients who need faster results, can take a week away from normal responsibilities, or haven’t responded adequately to standard TMS.
Stanford researchers developed the SAINT protocol after recognizing that depression is a medical emergency for many patients. Waiting six weeks for standard TMS to work isn’t acceptable when someone is severely depressed or experiencing suicidal thoughts.
The accelerated approach uses intermittent Theta Burst Stimulation (iTBS), a newer form of TMS that delivers therapeutic pulses in concentrated bursts. Each iTBS session takes only about three minutes compared to 19-37 minutes for standard rTMS, but research shows equivalent effectiveness.
A 2024 meta-analysis published in BMC Psychiatry examined accelerated TMS studies and found that depression scores significantly decreased following treatment, with no significant difference in effectiveness between accelerated and standard protocols. The analysis also found evidence of long-term maintenance effects, with depression scores at follow-up remaining lower than immediately post-treatment.
Stanford’s original research reported response rates approaching 90% with remission rates around 80% in treatment-resistant patients. A 2024 pilot study published in Brain Stimulation using a pragmatic accelerated protocol found 70% response rates and 55% remission rates at four-week follow-up, with effect sizes nearly three times the magnitude typically considered “large.”
Treatment Schedule:
A Typical Treatment Day: You’ll arrive in the morning and receive your first session. After each treatment, you’ll have about 50 minutes before your next session. Many patients work on laptops, make phone calls, read, or relax between sessions. Lunch is taken between sessions. You’ll finish in the late afternoon.
The five-day schedule typically runs Monday through Friday. You’ll need to arrange transportation for the treatment week since we recommend not driving during the intensive treatment period.
Timeline for Results: Many patients notice improvement within the first few days of treatment. Research shows depression scores typically decrease progressively each treatment day. Benefits are often well-established by the end of the five-day protocol and maintained at follow-up.
Accelerated TMS Long Island treatment is ideal if you need faster results than standard TMS provides, can dedicate one full week to treatment, have severe depression requiring rapid intervention, are traveling from out of state or internationally and can’t commit to six weeks of daily treatment, have tried standard TMS without adequate response, or work in a demanding profession where six weeks of daily appointments isn’t feasible.
The accelerated protocol requires more intensive time commitment during the treatment week but dramatically reduces total treatment duration. Instead of 36 visits over nine weeks, you complete treatment in five days.
Time Off Work: You’ll need to take the treatment week off from work or other major responsibilities. Many patients find this easier to arrange than six weeks of daily appointments that interrupt their schedule.
Accommodations: For patients traveling from outside Long Island, comfortable hotels are available near both our Great Neck and Mineola offices. We’re close to JFK and LaGuardia airports and easily accessible via Long Island Railroad.
Post-Treatment: After completing the five-day protocol, you can typically return to normal activities. We schedule follow-up appointments to monitor your response and determine if any maintenance treatment would be beneficial.
Accelerated TMS costs $12,500 for the complete five-day protocol. Most insurance plans don’t cover accelerated protocols yet, though this is changing as research continues to demonstrate effectiveness. We can provide documentation if you wish to submit for out-of-network reimbursement.
One-Day TMS (ONE-D) represents the most condensed treatment option available, delivering 20 TMS sessions in a single day combined with medications that enhance brain plasticity. This protocol is designed for patients who need the absolute fastest possible treatment timeline.
The ONE-D protocol builds on accelerated TMS principles but compresses treatment even further. You receive 20 iTBS sessions over the course of one day, spaced to allow optimal brain response between treatments.
What makes ONE-D unique is the addition of two medications that enhance neuroplasticity, your brain’s ability to form new connections. By temporarily increasing plasticity during intensive TMS delivery, the protocol aims to maximize the brain changes that produce antidepressant effects.
One-day TMS protocols represent the cutting edge of TMS research. While not yet FDA-approved as a specific protocol, the approach builds on established principles. Research has shown that multiple daily TMS sessions are safe and effective, neuroplasticity-enhancing medications can augment TMS effects, and compressed treatment timelines can produce rapid improvement.
Early studies of intensive single-day protocols show promising results, though this remains a newer approach with less long-term data than standard or five-day accelerated protocols.
Treatment Schedule:
The Treatment Day: You’ll arrive in the morning after following preparation instructions regarding the plasticity-enhancing medications. Throughout the day, you’ll receive iTBS sessions with breaks between treatments. Our team monitors you throughout the day and ensures your comfort. You’ll need someone to drive you home after treatment.
Timeline for Results: Some patients notice improvement within days of ONE-D treatment. As with all TMS protocols, full benefits may continue developing over the following weeks as brain changes stabilize.
One-Day TMS Long Island treatment may be right if you need the absolute fastest treatment timeline, are a busy professional who can’t take extended time away from work, are traveling from far away and have very limited time available, are comfortable with a newer protocol that has less long-term data, or want to combine TMS with plasticity-enhancing approaches.
Newer Protocol: ONE-D is based on sound scientific principles and emerging research, but it has less extensive data than standard or five-day accelerated TMS. We discuss this openly during consultation so you can make an informed decision.
Not FDA-Approved: While the individual components (iTBS and the medications used) are established, the specific ONE-D combination protocol hasn’t received separate FDA approval. This is common for innovative treatment approaches that combine existing approved elements in new ways.
Follow-Up: After ONE-D treatment, we monitor your response closely. Some patients benefit from additional sessions or maintenance treatment.
One-Day TMS costs $6,000 for the complete protocol. Insurance typically doesn’t cover this approach. We can provide documentation for out-of-network reimbursement submission if desired.

You’ll meet with one of our psychiatrists for a comprehensive evaluation. We’ll review your treatment history, discuss your symptoms and goals, and determine whether TMS is appropriate for your situation. If you’re a good candidate, we’ll explain the different protocol options and help you choose the best approach.
We’ll also verify your insurance coverage for standard TMS. Coverage typically requires prior authorization, which our team handles for you.
At your first treatment, we’ll determine your motor threshold, the minimum magnetic intensity needed to activate your motor cortex. This calibration ensures your treatment is personalized to your brain’s responsiveness.
You’ll sit in a comfortable chair while we position the TMS coil against your scalp. The device delivers magnetic pulses that feel like tapping on your head. Some patients find the sensation unusual at first, but most adjust quickly. You’re awake and alert throughout.
TMS has an excellent safety profile compared to medications. The most common side effect is mild scalp discomfort or headache during or shortly after treatment, which typically responds to over-the-counter pain relievers and often diminishes as treatment progresses.
Unlike antidepressants, TMS doesn’t cause weight gain, sexual dysfunction, emotional blunting, or withdrawal symptoms. It’s safe for elderly patients and doesn’t interact with other medications.
Benefits typically persist for months to years after completing TMS. If symptoms return, treatment can be repeated. Many insurance plans cover additional treatment courses after a period of sustained benefit. Some patients benefit from occasional maintenance sessions.

We’re one of the few practices offering standard, accelerated, and one-day TMS protocols. Whether you need insurance-covered treatment that fits around work or the fastest possible results, we have an option for you.
For standard TMS, our team handles benefits verification and prior authorization, working with your insurance company to maximize coverage and minimize out-of-pocket costs.
Results vary by protocol. Standard TMS typically shows improvement within two to four weeks. Accelerated TMS often produces noticeable improvement within the five-day treatment period. One-day TMS may show effects within days. All protocols continue building benefits after treatment completion.
TMS isn’t painful, though the magnetic pulses create a tapping sensation on the scalp that some patients find unusual at first. Mild scalp discomfort or headache can occur but typically diminishes as treatment progresses.
Yes, TMS can be safely combined with most psychiatric medications, and some antidepressants may actually enhance TMS effectiveness. We’ll review your complete medication list during your evaluation.
TMS benefits can last months to years after treatment completion. If symptoms return, TMS can typically be repeated. Some patients benefit from periodic maintenance sessions.
Most major insurance plans cover standard TMS for treatment-resistant depression. Accelerated and one-day protocols generally aren’t covered yet. Our team verifies your benefits and handles prior authorization for standard TMS.
Standard TMS delivers one session daily over six weeks (36 total sessions). Accelerated TMS delivers 10 sessions daily over five days (50 total sessions). Research shows similar effectiveness, but accelerated TMS produces faster results. Standard TMS is usually covered by insurance; accelerated TMS typically isn’t.
TMS isn’t appropriate for people with metal implants in or near the head (except dental fillings), cochlear implants, or implanted stimulators. People with a history of seizures require careful evaluation. We screen for contraindications during your initial consultation.
For standard TMS, yes. You can drive immediately after sessions. For accelerated and one-day protocols, we recommend arranging transportation during the treatment period due to the intensive schedule.
If antidepressants haven’t worked well enough, or if you’d prefer to treat depression without medication, TMS therapy on Long Island offers a proven alternative. Whether you choose standard TMS for insurance coverage and gradual improvement, accelerated TMS for rapid results in one week, or one-day TMS for maximum speed, we have an approach that fits your needs.
Call us at (646) 830-0131 to schedule a consultation. We’ll evaluate whether TMS is right for your situation, explain your protocol options, and help you make an informed decision.
